How to File a Formal Grievance under Section 504 and Title II
It is the policy of Johnston Community College not to discriminate on the basis of disability. Any person who wishes to submit a formal grievance/complaint regarding a violation of Section 504 or Title II related to the accessibility of any Web site that is maintained by the College may do so in writing or by telephone. Grievances can be submitted in writing via e-mail at firstname.lastname@example.org or by mailing it to: Johnston Community College, Senior Director of College Communications, 245 College Road, Smithfield, NC 27577. Grievances may also be submitted by telephone by contacting the Senior Director of College Communications at (919) 209-2563. The complainant should submit in their grievance/complaint a description of the problem, including:
Date of the Complaint
Description of problem encountered
Web address or location of the problem page
Contact information in case more details are needed (email/phone number)
The College will work to resolve the grievance/complaint in a fair and expeditious manner. Retaliation against any person who files a complaint of alleged discrimination or participates in an investigation of such complaint is prohibited under College policy.
